Please contact Dr. Takeuchi if you have any questions about current rates or would like to obtain a good faith estimate of costs. Important: If you intend to use your insurance (Aetna Behavioral Health, MHN), please contact your insurance carrier before the first session to ensure that my services are covered by your plan. Additionally, please check to see if Aetna or MHN is your source of primary insurance coverage in the event that you have double coverage with more than one insurer. CAPP is not contracted with Anthem Blue Cross, Blue Cross or Blue Shield.
